iSmile Dental performs prosthetics with subperiosteal implants, usually in cases where root structure installation is impossible. They are chosen mainly by patients who want to save time on bone augmentation or this process is impossible for medical reasons. The main area of application of subperiosteal implants is teeth restoration in the presence of too-thin bone tissue.
Features of Subperiosteal Implants
The installation of subperiosteal implants almost does not injure the preserved bone tissue. They are attached under the gum to the lateral surface of the jaw. Requirements for the size of the alveolar process with this method of prosthetics are minimal - its size should be at least 5 millimeters.
The advantage of subperiosteal implants:
- no need for bone augmentation and sinus lifting;
- ideal "landing" on the alveolar process since the systems are made individually;
- no risk of damaging large vessels and nerve bundles inside the jaw bone;
- qualitative distribution of chewing pressure from the prosthesis to the bone;
- quick installation of the prosthesis - either on the day of implantation or within five days, but can not thoroughly chew in the first weeks.
A subperiosteal implant is a thin, open metal structure with special supports protruding into the oral cavity. Both removable and non-removable dentures can be installed on the supports. Due to the large area, it is securely held on the gum and effectively copes with the assigned tasks.
Their peculiarity is that they take up a significant amount of space. Since implants are fixed not in the hard bone but on the periosteum, the massive size is justified - otherwise, the implants will not be able to fix themselves in the patient's oral cavity.
After the implants are placed, their tops remain on the surface so that the prostheses can be fixed immediately after the operation. At first - only temporary, created from light plastic. And after 2-4 months, they change to permanent.
As for materials, the main one is a cobalt-chromium alloy, but it is preferable to make implants from biocompatible titanium, which does not provoke allergies.
One-Step Method
There are two methods of installing subperiosteal structures - two-stage and one-stage.
Since the method of two-stage installation is additionally traumatic, with the development of visualization and 3D modeling in dentistry, this method is not used in iSmile Dental.
The one-stage method is less traumatic since it does not require preliminary surgical intervention to make a cast from the jaw bones. In this case, determining the size of the implant is carried out with the help of new digital technologies, in particular, research on a computer tomograph. The data obtained is downloaded to a computer, where a special program visualizes a 3D model of the jaw bone.
After that, the three-dimensional image simulates the shape and size of the implant, which subsequently, in a special laboratory, turns into a natural design that considers all the features of the jaw structure and fits it perfectly. After the form is made, it is time for the operation, during which the jaw is prepared, and a subperiosteal implant is installed on it. So modern digital technologies accelerate the process of prosthetics and reduce its traumatism.
Preparation Before Implantation
Before placing a subperiosteal implant, it is necessary to sanitize the oral cavity - cure the preserved carious teeth, remove tartar and plaque, and cure gum disease. Removing the collapsing roots is essential, which can become a source of infection and lead to further implant rejection.
The preparation before implantation also includes the delivery of tests to identify contraindications and the passage of an X-ray examination.
This type of implantation is performed under anesthesia.
Rehabilitation Period After Implantation
After the operation, you should not eat too hard food because the implants must be securely fixed on the jaw. Excessive load can lead to their displacement and even rejection. Therefore, products must be pre-crushed or kneaded if it is, for example, stewed vegetables.
To accelerate rehabilitation and eliminate complications, the dentist’s recommendations should be strictly followed – take prescribed medications, observe oral hygiene, and do not expose the body to physical exertion or temperature changes.

What is Subperiosteal?
The word “subperiosteal” is borrowed from Latin. It means that the prosthesis is carried out under the tissue of the periosteum. The periosteum is a special body tissue responsible for the growth of bones and their restoration in the event of fractures.
What can replace implants?
Removable dentures – clasp, flexible, acrylic, and non-removable products made of cermet, all-ceramic, and zirconium – dental bridges are able to replace implants, but this will be incomplete. A small gap from a missing tooth can be eliminated by displacing adjacent teeth with braces.
How long can I walk with a stitched implant?
You can safely walk with sewn implants until it becomes possible to do prosthetics. By the way, pregnancy is not a contraindication for prosthetics on implants.
What happens if you do not clean the implants?
Without proper oral care, the risk of inflammation of the soft tissues around the implant increases. This can lead to a more severe complication, bone resorption around the implant, and rejection of the structure in the absence of treatment.
